2. Print out the order form and fill it in

3. Price:

	UK - (GBP) 27.02 inclusive of all charges (including VAT).
	Rest of World - (GBP) 24.00 inclusive of all charges (Approx. exchange rate GBP 1 = US$ 1.95)

4. Method of Payment

UK - by cheque on a UK Bank made payable to 'Cavendish Information Products Ltd'(by post - enclose order do not fax) or by VISA or MasterCard (see below). If payment is being made by cheque please allow for 7 days clearance.
Rest of World - by International Money order in favour of 'Cavendish Information Products Ltd' (by post - enclose order do not fax) or by VISA or MasterCard (see below).

Please note that credit cannot be given as the cost of credit control exceeds the value of the ordered item.

Please ensure that you have filled in the following information:

1. Your name and address clearly in capital letters
2. Your email address and or fax number if available
3. Quantity of CD's required*
4. Total price payable*
5. Method of Payment (see below for VISA/MasterCard)
6. Your order number if applicable

*In the case of Bulk discounts please also give the bulk discount reference number for the agreed arrangement.

VISA or MasterCard
If using either of these credit cards for purchase please supply the following information:

1. Whether VISA or MasterCard
2. The credit card number
3. Security Code on card
4. The expiry date
5. The name on the card
6. Card issued by
7. The address of person to whom the card was issued if different to the address for delivery.
8. The amount
9. Payable to 'Cavendish Information Products Ltd'
10. Signature

There will be no deduction from the card until the goods have been dispatched.

It is regretted that no telephone orders can be accepted.
